St. Joseph’s Health Centre, Guelph works collaboratively with each of the 36 Long-term care homes in our area to operationalize the WW Behaviour Supports Ontario program. Each long-term care home hires their own BSO embedded staff. This multidisciplinary team can include nursing, PSW, and allied health (recreation and social work). St. Joseph’s works to ensure there is program consistency and standardization across the LHIN.Community Model
St. Joseph’s Health Centre, Guelph works collaboratively with the Canadian Mental Health Association Waterloo Wellington Dufferin to offer outreach Behaviour Supports to people in our Community. Healthcare providers in Family Health Teams, the Community Care Access Centre, Adult Day Programs, and other Community Support Services collaborate to ensure that the person with predictable, responsive behaviours and their family is supported in the most appropriate way.
